反超半个身位：KRAS G12C抑制剂Adagrasib获FDA加速批准治疗结直肠癌

新增适应症

2024年6月21日，美国FDA加速批准Adagrasib（商品名：Krazati，阿达格拉西布）新的适应症，联合西妥昔单抗治疗经治、带有KRAS G12C突变的结直肠癌（CRC）患者。这一批准标志着Adagrasib继2022年获批治疗局部晚期或转移性非小细胞肺癌（NSCLC）患者之后的又一重要里程碑。

目前FDA加速批准了两款KRAS G12C抑制剂用于NSCLC治疗，分别为Sotorasib和Adagrasib。Sotorasib联合帕尼单抗治疗结直肠癌的Ⅲ期临床结果已公布，但被Adagrasib反超半个身位。

 

Adagrasib新突破

本次Adagrasib获批用于联合西妥昔单抗（Cetuximab）治疗KRAS G12C突变的经治晚期或转移性结直肠癌（CRC）。在KRYSTAL-1试验中，94例患者接受了Adagrasib联合西妥昔单抗治疗。中位随访时间为11.9个月，客观缓解率为34.0%，疾病控制率为85.1%，中位缓解持续时间为5.8个月。

 

Adagrasib是一种特异性良好、不可逆的共价抑制剂，靶向KRAS G12C突变蛋白。Adagrasib选择性的结合并抑制KRAS活性，阻断癌细胞的信号通路，抑制其增殖和生长。在非小细胞肺癌和结直肠癌及其他实体瘤治疗，Adagrasib展现出良好的疗效。

基于Adagrasib在II期KRYSTAL-1临床试验中的结果，FDA于2022年12月12日加速批准其用于治疗KRAS G12C突变的局部晚期或转移性NSCLC患者。II期结果显示，Adagrasib的总缓解率为43%，疾病控制率为80%。中位随访12.9个月后，总生存期为11.7个月。同时，33%患有脑转移的患者的脑转移被缩小，显示Adagrasib具有良好的穿透血脑屏障的能力。

Adagrasib有望突破癌种限制。

2023年ASCO会议公布的Ⅱ期KRYSTAL-1数据显示，接受Adagrasib治疗的KRAS G12C突变实体瘤患者（n=57，不包含CRC和NSCLC）中，客观缓解率（ORR）为35.1%，疾病控制率（DCR）为86.0%，50.9%的患者病情稳定。
